Interested in receiving care from the Spine and Pain Center of Kauai, but not sure how to start? We’d love to help. Below is a step by step process of how it works!
1. Schedule Your First Appointment
When your physician refers you to our clinic or you contact us yourself, you will speak with our Patient Relationship Coordinator. They will schedule a time and date that works the best for you with one of our Pain Management providers. Following your initial conversation, the Patient Relationship Coordinator will mail you a New Patient Packet that contains more information about our practice and the forms you’ll need to fill out for your first appointment. If you have any questions about the forms or need help filling them out, please give us a call, we are happy to assist you!

2. Prepare For Your First Appointment
The day before your appointment, clinic staff will give you a reminder call. We will also inform you of the COVID-19 precautions that are in place for in-person visits. Please arrive 30 minutes prior to your scheduled appointment and be sure to bring the following:
• Your completed New Patient Packet
• A current medication list
• Your insurance card
• Recent imaging if you have it
• A form of personal identification with a photo on it (a driver’s license or ID card)
3. Arrive At Appointment

4. Receive Care
One of our friendly Medical Assistants will lead you to the exam room! They will take your vitals and discuss your reason for the visit. Your appointment with the doctor will last approximately 45 minutes. Following your consultation with your physician a scheduling specialist will set up your next appointment!
