How To Receive Care From SPCK

Interested in receiving care from the Spine and Pain Center of Kauai, but not sure how to start? We’d love to help. Below is a step by step process of how it works!

1. Schedule Your First Appointment

When your physician refers you to our clinic or you contact us yourself, you will speak with our Patient Relationship Coordinator. They will schedule a time and date that works the best for you with one of our Pain Management providers. Following your initial conversation, the Patient Relationship Coordinator will e-mail you a New Patient Packet at your request that contains more information about our practice and the forms you’ll need to fill out for your first appointment. If you have any questions about the forms or need help filling them out, please give us a call, we are happy to assist you!

*Our Patient Relationship Coordinator will be your partner through every step of the process, before, and during your first appointment. Whether you’re ready to schedule an appointment, or you simply have some questions, we are here to help. You may reach us at (808) 378-4439.

2. Prepare For Your First Appointment

The day before your appointment, clinic staff will give you a reminder call. We will also inform you of the COVID-19 precautions that are in place for in-person visits. Please arrive 30 minutes prior to your scheduled appointment and be sure to bring the following:

• Your completed New Patient Packet

• A current medication list

• Your insurance card

• Recent imaging if you have it

• A form of personal identification with a photo on it (a driver’s license or ID card)

3. Arrive At Appointment

*Our address is 3176 Poipu Road Suite #2, Koloa, Hawaii 96756. We are on the ground floor level.

Upon arrival please be sure to check in with the front desk and give your completed forms and other requested documents to the front desk scheduler. If you still have incomplete information in your new patient packet, please let staff know at this time so that they may help you complete it. You will be given a privacy policy form and a medication agreement form to read and sign. Please be prepared to pay your copay at this time.

4. Receive Care

One of our friendly Medical Assistants will lead you to the exam room! They will take your vitals and discuss your reason for the visit. Your appointment with the doctor will last approximately 45 minutes. Following your consultation with your physician a scheduling specialist will set up your next appointment!

Let us know if you have any additional questions; we are here to serve you!
